EPSDT Providers can go paperless beginning January 1, 2022

Dear Providers and Staff,

We wanted to share important information from Med-QUEST.  You can find their announcements and forms on their website, https://medquest.hawaii.gov/EPSDT.

  • Access DHS 8015 and 8016 Forms online
  • Submit forms online
  • Get paid faster by going paperless with electronic claims for EPSDT visits too

Starting January 1, 2022, Early and Periodic Screening, Diagnostic and Treatment (EPSDT) providers will have 2 new options to submit DHS 8015 and 8016 Forms online. Forms will be able to be submitted using an online form and using a fillable PDF.

Forms can be completed by logging into the portal to enter and submit EPSDT visit data. Some benefits with the new online form include:

  • Easy steps to complete rather than handwriting on paper forms.
  • A guided experience to easily navigate to the next or previous steps.
  • Use the navigation to move between the sections of the form.
  • Enter information, verify, confirm, and submit within minutes.
  • No need to send it in the mail!
  • Have claims processed and paid faster.

Med-QUEST (MQD) is also updating the EPSDT periodicity schedule to align with the American Academy of Pediatrics (AAP) Bright Futures Periodicity Schedule. Annual visits are encouraged and covered under the EPSDT benefit. The new schedule adds EIGHT reimbursable visits at 30m, 7y, 9y, 11y, 13y, 15y, 17y, and 19y.

This means the new periodicity schedule allows reimbursable visits at the following ages:

  • <1m
  • 1m
  • 2m
  • 4m
  • 6m
  • 9m
  • 12m
  • 15m
  • 18m
  • 24m
  • 30m (new!)
  • 3y
  • 4y
  • 5y
  • 6y
  • 7y (new!)
  • 8y
  • 9y (new!)
  • 10y
  • 11y (new!)

New DHS 8015/8016 forms will reflect the new periodicity schedule and be available online.

The goal of going paperless is to reduce unnecessary provider burden and improve how we serve our communities. For consistency, MDQ and QUEST Integration health plan will only accept the new DHS 8015/8016 for EPSDT visits occurring on or after January 1, 2022.

Note: Print options will still be available for EPSDT providers who are not ready to use online forms now.
